Sick Note requests can be submitted via email or eConsult. If you send in your request via email, please provide a brief reason you need the Sick Note, the dates you need the Sick Note to start and end and if the request is for a new sick note or an extension.
If you have been ill for 7 days or less, you don’t need to speak to a Doctor. You can complete a Self-Certification form yourself. Many employers have their own self-certification forms. If your employer doesn’t have its own form, please see the link below for more information.
If you have been ill for more than 7 days, you will need a doctor’s certificate. These certificates are called ‘Fitness to Work’ Certificates (Sick/Fit Note). The doctor may need to speak to you before a Sick Note can be issued. If you have already been seen by your doctor about this illness a Sick Note will be sent to you within 5 working days of your request.
If you have already had a Sick Note for this illness your Doctor may not need to speak you to issue an additional Sick Note. If the Doctor needs to speak to you, we will contact you, if not you will be sent your Sick Note within 5 working days of your request.
We do not take requests for urgent Sick Notes as a Sick Note start date can be back dated. Sick Notes cannot be forward dated so please do not send in requests for a Sick Note extension until the previous sick note has expired. Please the link below for further information on Sick Notes.
