The Same Day Access Service operating from Fareham Community Hospital was implemented as a pilot scheme in September 2017 with three local practices, Highlands, Whiteley and Jubilee participating. As was widely publicised Stubbington Medical Practice joined the service a little later, on 2 January 2018.
Unfortunately the introduction of Stubbington Medical Practice into the hub model has produced some unexpected and unforeseeable technical issues. Those issues may take a while to resolve and so from 8am on Wednesday 17 January 2018 Stubbington Medical Practice will withdraw from the pilot scheme and revert to its former practice of managing same day Acute Services at the practice premises.
Stubbington Medical Practice remains very supportive of the pilot scheme’s motives and principles and looks forward to the possibility of a reintroduction to the service in the future. We will continue to keep you informed as developments arise.
Patients should continue to contact the practice as usual throughout the transition. We will be continuing the good work with our Care Navigators as this has enabled us to improve the efficiency of the triaging system. Thank you for your co-operation in answering our care navigators' questions when you contact the practice.
